Please let us know prior to your visit if you have a medical condition that may require special assistance or need to take medication prior to dental cleanings (i.e. antibiotics for premedication due to endocarditis or joint replacement)
IMPORTANT: A parent or guardian must accompany all patients under 18 at the initial visit.

Billing & Financing
Payment for services is due at the time services are rendered. For patients with insurance, this includes any co-payments and deductibles required by the insurance carrier. We accept cash, checks, Health Savings Accounts (HSA), Flexible Spending Accounts (FSA), Visa, Discover and MasterCard.
Cancellation Policy
New Street Dental requires at least three business days’ notice for rescheduling appointments to ensure high-quality care and to manage operational costs. A $150 fee will be charged for cancellations or reschedules made with less than three business days’ notice. The practice appreciates understanding and support in maintaining its personalized service. By scheduling an appointment, you agree to abide by this policy. Insurance
If you have dental insurance, we will help you to receive your maximum allowable benefits and, as a courtesy to our patients, our office will bill your insurance company. Most insurance companies will respond within four to six weeks. Please call our office at (831) 426-1343 if your statement does not reflect your insurance payment within that time frame. Please bring your insurance card or policy number and insurance company phone number with you at your first visit.
